Gordon, you talk a lot about simplicity and keeping thing simple, can you tell us why you think that is important?

Gordon one of your key principles is accountability; can you tell us what the difference is between being accountable and being responsible?

You mention that in order to be successful, we need to improve our effectiveness and efficiency can you tell us the difference between those.

Gordon, there are some great examples of failures in the book, where you highlight what can go wrong when you lack failure, can you share with us your favorite.
